Our Care8 Approach to Home Care Services
DIAL-AN-ANGEL’s Care service principles have evolved since 1967. We understand that everyone has different aspirations, preferences and goals, so we tailor our services specifically to your needs by using a person-centred care approach called Care8™.
This aligns with the concept of Consumer Directed Care (CDC) and The Australian Government’s new Home Care Packages, as part of its Living Longer Living Better reform package. A Home Care Package provides services that will help you to remain at home for as long as possible, as well as providing choice and flexibility in the way that the care and support is offered.
The goal of our Home Care program is to assist our clients to remain living independently in their home, giving the client the choice and flexibility on how that care and support is provided.
The DIAL-AN-ANGEL Care8 model of assessing, quoting, personalising, collaboratively reviewing and evaluating our services for the care-recipient highlights the person-centred philosophy practiced by our Agency. We have a skilled care workforce who are trained to identify and present suggestions for continuous improvements to the care recipient’s individual program.
Care 1 - INITIAL CONTACT
DIAL-AN-ANGEL® Coordinator estimates your care level, discusses the situation and your Service Rights and Responsibilities with you or your representative.
Care 2 - FORMAL ASSESSMENT
We help you obtain your “Care Assessment” via our Registered Nurses, together with, or by your Health Professionals or ACAT. We collect your desired “Outcomes”
Care 3 - QUOTE PROPOSAL
Using the information we have collected from you we prepare a “Care8 Quote Kit” which demonstrates the choices available to you for care in the home.
Care 4 - CUSTOMER-CENTRED GOAL-SETTING CHOICES
Your Case Coordinator will deliver your “Care8 Quote Kit” which includes your customised “Care8 Goal Plan” and at this stage we will normally complete a “ Home Safety Checklist” at your home.
Care 5 - HOME CARE AGREEMENT
The “Service and Goals Agreement” is a written agreement prepared from the adjusted Care8 Quote that your or your representative complete and sign. Your Case Coordinator will create your file for the Angel/s that are matched for your plan.
Care 6 - HOME COMMUNICATION FOLDER
We will prepare your “Site-Care Folder ” including Individualised Care and Goals Actions Plan and Care Shift Notes and commence support for you.
Care 7 - REASSESSMENT and REPORTING
We will diarise observations using Care Shift Notes and produce a regular financial statement and full reporting for reassessment.
Care 8 - EVALUATION
Following Reassessment, we seek your rating of our support for your Care and Goals. This is your Care-Rating of DIAL-AN-ANGEL and an opportunity to revisit your estimated level of care from Care 1

DIAL-AN-ANGEL is a highly qualified and professional care provider with certification in a Quality Management System known as ISO:9001 as well as being a listed provider under the National Disability Scheme for the three launch sites.
We have set out our Charter of Rights and Responsibilities for Service Users of Attendant Care Services here, which we believe, should be the minimum entitlement for each of our service users
DIAL-AN-ANGEL looks forward to utilising its approval as a services provider by the Department of Health and Ageing prior to Consumer Driven Care reforms of the Living Longer Living Better legislation.
DIAL-AN-ANGEL also provides Angels for housekeeping & home cleaning, gardening & home maintenance, nannying and babysitting, supervised contact visits, party help, aged care, in-home nursing, dementia, palliative and disability care in the home.
